Attention Patients of the Central Lambton Family Health Team attending College and University FALL 2015, particularly in Health Sciences and Social Services Programs

We are pleased to offer a school forms completion clinic which entails

  • Step 1 and 2 TB Skin Testing (as requested by school)
  • Necessary required lab work showing proof of Immunity to communicable diseases such as Hepatitis B, Measles, Mumps and Rubella, Varicella (chicken pox) (as required by school)
  • Additional counselling on new vaccines and immunization updates.
  • Completion and sign off of forms to allow for clinical or co-op placements as required by School.Please contact the Central Lambton Family Health Team, Blanche Street site at 519-882-2500 to book your appointment to get started. IMPORTANT NOTE: The process of TB skin testing, additional lab work, and form completion take an average of 3-4 weeks to complete. The process will take approximately 3-4 short appointments. Please make your appointment as soon as information and forms are released from your school that you will be attending in the fall. Do not delay completion towards the end of the summer as forms will not be done on time for the start of school and clinical placements. We will not be held responsible for incomplete forms if the initial appointment is not made by Monday August 10, 2015.  Please come prepared with your current immunization record, and your section of your school form completed. A standard charge of $35 dollars is required to have TB skin and lab tests and forms completed.

Children attending elementary school and secondary school are also encouraged to make sure their immunizations are up to date. Lambton Public Health states,As of July 1, 2014 school-age children now require proof of immunization against meningitis, pertussis (whooping cough) and for children born in 2010 or later — chicken pox, to attend school. This is in addition to existing requirements for proof of immunization against diphtheria, tetanus, polio, measles, mumps and rubella. Children and teenagers who have received all recommended vaccines as per Ontario’s immunization schedule will not require any further immunizations.” If your child/children require vaccines to catch up prior to the start of school, please call the Central Lambton Family Health Team at 519-882-2500.
