Dear Patient,

The Central Lambton Family Health Team (CLFHT) has begun developing a new 3-year strategic plan to help us continue to provide excellent, patient-centered care and improve the health of our community. This plan will only be successful if we have your valuable input.

We invite you to share your thoughts and experiences through this brief survey. Your feedback will help us identify areas of excellence and opportunities for improvement, directly shaping our future goals and priorities.

Your participation is entirely voluntary, and your responses will be kept confidential and anonymous. The survey should take 5-10 minutes to complete.

An additional in-person feedback session led by an independent facilitator will also take place on Monday January 19, 2026. There will be 2 sessions to choose from, either 1:00-3:00 pm or 5:30-7:30 pm. Food will be provided.
