Tuesday, December 11th, 2018 4:30-6:30 pm.
This opportunity to meet Dr. Cooper and her family will be held in the lobby at
4130 Glenview Road, Unit 3, Petrolia.
If you have not already completed your Ministry of Health Enrollment form with
Dr. Cooper, enrollment forms will be available.
Reminder, all patients are required to complete new enrollment forms to ensure your continued enrollment with Dr. Cooper
