How to become at Patient at CLFHT

Physicians at 4130 Glenview Road site of the Central Lambton Family Health Team will not be accepting patients until further notice.

Over the past three years, physicians have taken on more than 4,000 new patients which has led to their practices currently being filled.

At this time, physicians are focusing on balancing the care of their existing clinic patients along with their additional responsibilities such as emergency room coverage, inpatient hospital care and nursing home care.

As the ability to accept new patients returns, announcements will be made notifying the community.

Central Lambton Family Health Team wishes to thank residents of Lambton County and our community partners for their ongoing support as we strive to provide the best possible care to patients.

Pick up an enrollment package at office, which consists of 4 documents

  • MOHLTC Enrollment form,
  • Patient Medical Information Sheet,
  • Request for Medical Records (from previous physician)
  • Emergency Contact and Pharmacy information.
  • Each person age 16 or over must complete their own set of forms.

Children under the age of 16 may be included on parent/guardian's MOHLTC Enrollment Form and must have their own Medical Information Sheet, Request for Medical Records, and Emergency Contact Information Sheet.

These packages can be completed in office or may be taken home for completion.

All patients registering must present a VALID health card, with the completed packageĀ at the time of registration.

Patients will be scheduled for a new patient appointment, at a later date.
